Join our team as an MDS Coordinator at Elkader Care Center

$37 an Hour

We are currently seeking a dedicated and experienced MDS Coordinator to join our team at Elkader Care Center located in Elkader, IA. The MDS Coordinator is a vital member of our interdisciplinary team, responsible for assessing residents and developing care plans to ensure they receive the highest quality care.

• Coordinate the development and completion of the MDS for each resident
• Collaborate with healthcare providers to develop comprehensive care plans
• Ensure accurate and timely documentation of resident assessments and care plans
• Maintain compliance with state and federal regulations regarding MDS assessments
• Participate in interdisciplinary meetings to discuss resident progress and adjust care plans as needed

• Active RN license in the state of Iowa
• Prefer 2 years experience as an MDS Coordinator in a long-term care setting
• Strong attention to detail and organizational skills
• Excellent communication and interpersonal skills
• Ability to work collaboratively in a team environment
