Nurse Practitioner – Niazi Family Medicine | Sacramento, CA

Summary:

Niazi Family Medicine is seeking a dedicated and compassionate Nurse Practitioner (NP) to join our patient-centered practice. The ideal candidate will provide high-quality primary care to a diverse population, with a strong preference for those who are fluent in Hindi, Farsi, Pashto , or other languages spoken in our community. You will play a key role in advancing our mission of inclusive, accessible care for all.

Success in this role looks like building trusted relationships with patients, delivering evidence-based care, and working collaboratively within a supportive team environment.

Responsibilities:

• Provide comprehensive primary care services, including physical exams, health assessments, and management of acute and chronic conditions
• Order, perform, and interpret diagnostic tests and lab results
• Develop and implement individualized treatment plans
• Prescribe medications and therapies in accordance with state and federal regulations
• Counsel patients on preventive care, lifestyle modifications, and treatment adherence
• Maintain thorough and timely electronic medical records (EMR)
• Collaborate with physicians, PAs, nurses, and administrative staff to ensure continuity and quality of care
• Foster a culturally sensitive environment by effectively communicating with patients from diverse backgrounds
• Participate in clinic quality initiatives and team meetings

Qualifications:

• Current and valid Nurse Practitioner license in the State of California
• Completion of an accredited Nurse Practitioner program
• Board certification (FNP or ANP) required
• Fluency or strong proficiency in Hindi, Farsi, Pashto , or other community languages is highly preferred
• Excellent clinical judgment and diagnostic skills
• Strong communication, organizational, and interpersonal abilities
• Experience in primary care, family medicine, or community health preferred
• Commitment to providing inclusive, patient-focused care
